Acquired angioedema

Fisiopatologia: consumption of C1q by Ag-Ab immune complexes binding, with subsequent consumption of C1-INH
  • Tipo I - associated with lymphoproliferative disorders (B-cell lymphoma, CLL, monoclonal gammopathy, breast cancer)
  • Tipo II - extremely rare autoimmune form with C1-INH autoantibodies, onset middle age
 
Exames complementares
  • Hemograma, bioquímica, velocidade de sedimentaçãi, urina
  • Complemento: pedir C3 e C4
    • C4 baixo entre e durante os episódios. Se normal considerar outro diagnóstico. Se anormal pedir níveis e exame funcional de C1-INH
  • Triptase se anafilaxia pode estar presente
  • C1 inhibitor antigen and function
 
Tratamento (não é a dermatologia)
  • Concentrado de C1 esterase inhibitor if available (Fresh frozen plasma if not)
    • Berinert
    • Cinryze
  • Kalbitor (Ecallantide): reversibly inhibits plasma kallikrein therefore decreasing bradykinin levels (caro)
  • Intubation
  • Volume support
  • On discharge
    • Start prophylaxis ideally with C1 esterase inhibitor
    • Refer to allergy/immunology for care
  • Androgénios eram usados antigamente mas já não se usa devido ao risco de carcinoma hepatocelular
