Lichen Striatus | Blaschkite | BLAISE (Blaschko Linear Acquired Inflammatory Skin Eruption)

Etymology
  • Saurat: Lichen striatus is incorrect terminology because it is not a lichen and striated refers to a surface with shallow grooves which is not the case
 
Epidemiology
  • Primarily children 4 months - 15 years
  • Female to male ratio 1,6-2:1
  • Seasonal variation: spring and summer
 
Etiology
  • Somatic mosaicism
    • Gene unknown
    • Keratinocytes migrate along the lines of Blaschko
    • Association with atopy → is an atopic diathesis
  • Triggering event makes the mosaic to reveal itself
    • Virus? May explain seasonal variation
    • BCG vaccine?
  • → induces aberrant clone of keratinocytes to produce a novel surface antigen
 
Clinical presentation
  • Continuous or interrupted band composed of discrete or clustered pink, skin-colored or tan (hypopigmented) papules that are flat-topped, smooth or scaly
  • Size 2-4mm
  • May be pruritic
  • Follow lines of Blaschko
  • May involve nail: onycholysis, splitting, fraying and total nail loss
 
Histology
  • Padrão de dermatite de interface liquenoide
  • Periadnexal inflammation: Envolve foliculo piloso e glândulas sebáceas
 
Tratamento
  • Usually not needed
  • Self limited - resolves within 1-2 years
  • Topical corticosteroids under occlusion
  • Topical calcineurin inhibitors
