Terapia Fotodinâmica (PDT)

Principle: the aim of dynamic phototherapy is the selective destruction by light of cells that have accumulated a photosensitizing substance.
  • The selectivity of the destruction is based on the preferential uptake of this substance by cells that are distinct from normal cells due to their metabolic and proliferative state
 
Mechanism
  • Activation of a topical photosensitizer by light
    • 5-Aminolevulinic acid (ALA) activated by blue light
    • Methyl aminolevulinate (MAL) activated by red light. This is more lipophilic and there’s occlusion recommended
  • They are preferentially absorbed by neoplastic cells (aktinic keratoses, non-melanoma skin cancer), cutibacterium acnes (acne)
  • Inside the cells, they are converted to protoporphirin-9, que tem 2 picos de absorção
    • Principal: na banda de Soret (azul)
        • Soret band is 400-410nm (low UVA and high visible - blue)
          • Can penetrate window glasses
          • Need physical blocking sunscreens (specially those containing iron oxide)
          • “Usual” UVA and UVB sunscreens will not block this!!!
    • Acessório nos 635 nm (vermelho)
  • A acumulação destes fotossensibilizadores nas células neoplásicas permite que a fototerapia as mate preferencialmente
